PARENT(S)/CARER AGREEMENT (by booking, you agree to have read this and agreed to this Membership Terms and Conditions • Out-of-School Clubs payments are on 4-weekly payment, invoice will be issue in advance. Full payments are required before the child’s commencement at the club. Monthly Invoice will be sent on the 7th of a month, payments must be made on receipt of invoice or within 14 days from the issued date, otherwise a late payment charge will be applied five days after due date. • In a case where we have to close the club due to an issue by us, any payment already made within the closure period will be carried forward to the next bill. • Let us know as early as possible if your child will not be attending the facility, please note that there is no refund for not attending. • Flexible bookings are welcome subject to availability. • A month notice is required for changes in attendance or for cancellation. • Children are to be dropped off at our setting in the morning and picked up in the evening only by an authorised person known to our member of staff. No other person will be allowed to pick the child without prior authorisation to My-STS management by the parent or carer. • Parents consent is required for our staff to collect children from the school and parents must inform the child’s school for us to collect their child/children from the school. • We close at 6 pm exactly, children must be collected by 6 pm as we have to leave the building at exactly 6 pm for the next user. If a child is not collected by 6 pm, and we have to stay outside longer for collection after our closing time, a further £5 will be charged every 10 minutes up till the child is collected. We will not like to apply these additional costs, so please collect your child on time and contact us as soon as you know that you’re running late. • Please inform us if your child is unwell but strong enough to attend school so that we can keep an eye on your child. • We expect good behaviour from all children and we will support children good behaviour. Any significant behaviour incident will be recorded and reported to the parent. And where the child behaviour does not improve despite the best efforts of the staff and parents, the management reserve the right to withdraw the children. • In addition to their school uniform, children should put on outfits appropriate for the weather condition of each season, for example, coat for winter/rain. • Children can bring their homework to complete.
